Ding dongs at the insurance company have twice now arbitrarily ignored the information in the submitted superbill and decided to pay a firm that my provider hasn't worked for since 2018 (they went solo practice). They also ignored that the superbills show that I paid the amounts in full so they should have sent me the checks anyway. Over an hour on the phone trying to fix the latest bill and somehow I expect they'll keep making this same mistake in the future.
